Many women complain about diet-resistant fat deposits in the area of their waist/riding breeches and at the same time are not satisfied with the size of their buttocks. The combination of liposuction of the flanks and fat injection into the buttocks can achieve results that could never be achieved with exercise.
Butt augmentation with autologous fat (Brazilian butt lift)
When sculpting the buttocks region, two aspects are always in the foreground: Firstly, many patients want an optimized view of the buttocks from behind, i.e., a straight line (hourglass shape) instead of unsightly bumps. In these cases, the excess fat deposits, particularly in the waist and saddle bags, are removed and, depending on the depth of the "hip dip", injected as an autologous fat treatment.
On the other hand, many patients want a rounder bottom from the side (90° perspective). In these cases, the upper area of the buttocks is filled with the patient's own fat, resulting in a rounder, beautifully shaped bottom.

Another example of a patient (35 years old) who had fat liposuctioned from her waist and injected into her buttocks. The entire body line looks much more harmonious and feminine as a result.
The treatment focuses on planning the procedure, which is always individually tailored to each person, their anatomy and wishes. First, excess fat is suctioned out, with particular attention paid to ensuring that the fat on the flanks and waist is completely removed. This sculpting alone changes the body silhouette above the buttocks in such a way that the waist becomes narrower and the curves of the buttocks are emphasized. At the same time, the transition from back to bottom is "defined". The aim of liposuction is not only to harvest fat for buttock augmentation, but also to optimize the contour at the same time. In most cases, the waist is significantly narrowed to emphasize the female figure.
The patient's own fat is used to sculpt the buttocks: The lateral regions on the buttocks, which appear depressed ("hip dips"), are usually filled in as well as the central buttocks region, which is responsible for the bulging of the buttocks. This combination leads to the desired rounding of the buttocks, also known as the "Brazilian butt".
In the LIPOCLINIC, the fat is injected only superficially – i.e., between the skin and the muscles. Compared to other methods, this is harmless and medically safe. We carry out the procedure exactly as it is specified by the Board of Plastic Surgeons in their guidelines.
Waterjet liposuction is ideal as it achieves the best ingrowth rate of up to 70%. Some of the implanted fat is broken down by the body; the fat that is still present after 4-6 weeks remains forever. Depending on the fat deposits, 200-500ml are implanted per session, but it is also possible to inject larger amounts using the so-called "supercharged" method. However, this is only possible if someone has a BMI of over 26. Injecting fat with vibrating cannulas is the new standard worldwide and prevents fat embolisms and other problems. This technique has been certified as very safe.
When implanting large amounts of fat – e.g., for a Brazilian Butt Lift (BBL) – 2-3 liters of fat are injected into the buttocks. A butt lift with such large amounts of fat can only be performed if the fat is injected not only between the skin and muscles, but also deep into and between the muscles. And this is where the problem begins: Fat can enter the bloodstream during such treatments, which can be very dangerous. At the LIPOCLINIC, we do not carry out such treatments; we implant a maximum of 500ml per side.
The treatment is performed on an outpatient basis in twilight sleep and under local anesthesia. You can go home immediately after the procedure, return to work two days later and then return to sport for two weeks. If the result does not meet your expectations after one session with autologous fat, a second treatment ("refill") can be carried out or the buttocks can be built up further with polylactate (Sculptra, Lanluma).

You can eat and drink normally before the procedure, our "short intoxication" has nothing to do with a normal anesthetic.
You can expect to spend 3 hours with us. This means a welcome, explanation of the procedure, treatment, dressing, short rest with drinks and farewell. Due to the twilight sleep, you will need to be picked up.
The fat removal area is pre-treated with a tumescent solution (local anesthetic). The fat is then removed using the gentle WAL procedure before being injected into the desired areas of the body.
Subsequently, "fate" determines how much fat survives the treatment. Every fat cell that survives will remain there forever – the result is therefore permanent. Individual life circumstances, smoking, genetics etc. influence the growth of fat and stem cells. It is therefore impossible to predict in advance how much fat will be reduced and how much fat will remain in the long term, as every body reacts differently. For this reason, it can sometimes take several treatments to achieve the desired curves.
In contrast to silicone implants, where the size remains the same forever, with autologous fat, it can unfortunately never be guaranteed how much volume will grow in – in individual cases, this can also mean that virtually no fat grows in.
Please note: We often remove fat from the knee, abdomen or hip area. Therefore, please bring wide trousers or a loose skirt with you so that you can easily put the thick bandage under your clothes after the procedure. You should also bring warm socks and a dark T-shirt, just in case.
If you would like to have your own fat injected into your face during the same session, you do not need to worry about any significant side effects such as discoloration (haematomas). If necessary, you can also immediately cover the injected areas with make-up.
A follow-up check is advisable after 4-8 weeks, when the result is assessed and further treatments are planned if necessary. Depending on the desired result, it may take several sessions to achieve the desired curves.
The result is not always satisfactory after a single treatment. Depending on the desired result, further procedures may be required before the expected result is achieved. Significant complications such as melting or nodule/cyst formation rarely occur because the quality of the fat and the injection technique have improved considerably, which also explains the high growth rate. Should such a change occur, it can be surgically removed at any time. Swelling and slight bruising are always to be expected, but these are usually no longer visible after a few days. As the treatment takes place under the skin, you can return to the sun immediately. A real "time-out" is not necessary, as you will always remain socially acceptable with our technique. Infections (inflammations) are also very rare because an antibiotic is given as a preventative measure with every treatment, which virtually rules out infections.

Some fat cells do not survive the transfer and are broken down by the body. MRI scans taken before the procedure and again six months later show that an average of 76% of the fat transferred using the WAL method remains. However, this is a statistical value – there are rare cases in which very little or no fat integrates. Depending on the treatment goal, a second or sometimes even third session may be needed to achieve the desired buttock volume. Satisfaction with this method in our clinic is excellent, with over 90% reporting positive results. The final outcome is visible after 6 months.
Infection
In rare cases, infection can occur shortly after surgery. However, because we administer antibiotics before and after the procedure and the WAL method uses a closed, sterile system, we have never seen an infection in our patients throughout all the years of performing this method. Should an infection occur, it usually heals quickly with antibiotic treatment. Only in extremely rare cases could an abscess form, which may require surgical intervention.
Oil cysts
In rare cases, some of the injected fat may not integrate and may partially liquefy. This can manifest as small oil cysts or, in some cases, as firm areas (microcalcifications) with discoloration. These side effects are harmless and can be removed surgically if bothersome. Thanks to the WAL method, such issues have become extremely rare.
The WAL method automatically transfers millions of stem cells from the fat tissue. This enrichment is one of the many advantages of this technique and contributes to the high amount of fat that successfully integrates.
The fat that integrates into the buttocks is permanent. Initially, the result appears “overcorrected” because a portion of the fat will naturally be resorbed. This process typically takes 4–8 weeks. The results visible after that period are long-lasting.
From our perspective, the WAL method is currently the best technique for treating large areas with fat transfer. It is a standardized method used by thousands of doctors worldwide. No other fat transfer technique is performed as frequently. This wide usage has led to continuous refinement, resulting in a highly optimized system. WAL is also the only method whose quality is controllable due to its standardization – including the machine, water-jet technology, container and cannula design, and pressure settings. Because of this, the outcomes are reliable regardless of a doctor’s individual experience, as there are very few unpredictable variables.
As the most widely used method for large-area fat transfer, the ongoing collective experience has further enhanced its technical precision and performance.
On the day of surgery, the areas for liposuction are marked. The procedure is performed under twilight sedation with local anesthesia, making it completely pain-free.
At the end of the procedure, the buttocks and liposuctioned areas are wrapped with a bandage, which you can remove the following day. You may return home immediately after the procedure and typically resume work two days later. In the days following, the buttocks will be swollen and slightly bruised, but significant pain is not expected. The liposuctioned areas may feel sore, often compared to muscle soreness.
Bruising and swelling generally subside within three weeks. Depending on the area treated, a compression garment must be worn for one week.
A follow-up check is recommended after about 4 weeks, with final results visible after 3 months. However, major changes no longer occur beyond week 6 – around 80% of the final result is visible by then. Sleeping on your back or side is usually not a problem, as long as you're used to it.
